Kindergarten Registration
Your child must be five years
of age by August 31, 2009 to register.
Items needed for registration include:
1. A certified copy of your child's birth certificate
2. An immunization record for the child
3. Proof of residence (housing contract, rental
agreement, utility bill - gas, water or light)
4. Picture ID of registering Parent/Guardian

| A kindergarten Health Assessment needs to be completed before the beginning of the school year. NC law allows the health assessment to occur between August 27, 2008 and the beginning of school in the year 2009. You will receive the form to be completed by your physician when you register your child at school. The completed form needs to be returned to school on or before the first day of school. |
